Cost by Diagnosis Related Group: Outputs from the First Phase Cost per Disease Project

บทคัดย่อ

การศึกษานี้มีวัตถุประสงค์เพื่อนำเสนอต้นทุนบริการผู้ป่วยในรายกลุ่มวินิจฉัยโรคร่วม (diagnosis related group, DRG) จากข้อมูลผู้ป่วยรายบุคคล แหล่งข้อมูลที่ใช้คือข้อมูลต้นทุนผู้ป่วยในรายบุคคล ของโรงพยาบาล 40 แห่ง ที่เข้าร่วมโครงการวิเคราะห์ต้นทุนรายโรค ระยะที่ 1 รวม 4 ปีงบประมาณ (2561-2564) จำนวน 2,263,459 ราย ซึ่งเป็นข้อมูลต้นทุนผู้ป่วยรายบุคคลในมุมมองของผู้ให้บริการ (provider perspective) ใช้วิธีการคำนวณต้นทุนบริการทางการแพทย์แบบมาตรฐานจากบนลงล่าง (top-down method) วิธีต้นทุนจุลภาค (bottom-up method) วิเคราะห์ต้นทุนรายกลุ่มวินิจฉัยโรคร่วม เฉลี่ยต่อรายและต้นทุนรวม (ต้นทุนเฉลี่ยต่อราย x จำนวนราย) ตามกลุ่มวินิจฉัยโรคร่วม
โดยก่อนการวิเคราะห์ได้ตัดข้อมูลต้นทุนที่ผิดปกติออก คงเหลือข้อมูลในการวิเคราะห์ 2,191,117 ราย (ร้อยละ 96.8) ผลพบว่า หากไม่รวมผู้ป่วยในที่เป็นโรคติดเชื้อไวรัสโคโรนา (DRG 23540, 04520, 04521) โรคที่พบบ่อย 3 ลำดับแรก คือ กลุ่มผู้ป่วยทารกแรกเกิด DRG 15540 Newborn admission weight >2499 gm without procedure without significant cost and clinical complexity มีต้นทุนเฉลี่ยต่อราย 5,192 บาท ต้นทุนรวม 599.2 ล้านบาท การผ่าตัดคลอด DRG 14010 Caesarean delivery wo sig CCC 18,954 บาท ต้นทุนรวม 1,248.1 ล้านบาท และ การคลอดปกติ DRG 14500 Vaginal delivery wo sig CCC 15,810 บาท ต้นทุนรวม 889.1 ล้านบาท โดยโรงพยาบาลขนาดใหญ่ (300 เตียงขึ้นไป) มีต้นทุนเฉลี่ยต่อรายในการรักษากลุ่มผู้ป่วยทารกแรกเกิด DRG 15540 Newb adm wt >2499 gm wo proc wo sig CCC 5,187 บาท การผ่าตัดคลอด DRG 14010 Caesarean delivery wo sig CCC 18,704 บาท และ การคลอดปกติ DRG 14500 Vaginal delivery wo sig CCC 15,465 บาท โรงพยาบาลขนาดเล็ก (น้อยกว่า 300 เตียง) กลุ่มผู้ป่วยทารกแรกเกิด DRG 15540 Newb adm wt >2499 gm wo proc wo sig CCC ต้นทุนเฉลี่ยต่อราย 5,216 บาท การคลอดปกติ DRG 14500 Vaginal delivery wo sig CCC 17,556 บาท โรคติดเชื้อระบบทางเดินอาหาร DRG 06570 Infectious gastroenteritis age > 9 wo sig CCC 5,654 บาท
          ผู้ป่วยเด็ก (แรกเกิดถึงอายุไม่เกิน 15 ปี) โรคที่พบบ่อย 3 ลำดับแรก คือ กลุ่มผู้ป่วยทารกแรกเกิด DRG 15540 Newb adm wt >2499 gm wo proc wo sig CCC ต้นทุนเฉลี่ยต่อราย 5,192 บาท ต้นทุนรวม 599.2 ล้านบาท รองลงมา คือ โรคระบบทางเดินอาหารอักเสบ DRG 06570 gastroenteritis age < 10 wo sig CCC 5,360 บาท ต้นทุนรวม 114.4 ล้านบาท และโรคติดเชื้อที่ จมูก ปาก คอและกล่องเสียง DRG 03520 nose, mouth, throat and larynx diseases wo sig CCC 8,059 บาท ต้นทุนรวม 121.4 ล้านบาท
          ผู้ป่วยวัยแรงงาน (อายุ 15 ถึง 60 ปี) การผ่าตัดคลอด DRG 14010 Caesarean delivery wo sig CCC มีต้นทุนเฉลี่ยต่อราย 18,954 บาท ต้นทุนรวม 1,246.1 ล้านบาท รองลงมา คือ การคลอดปกติ DRG 14500 Vaginal delivery wo sig CCC 15,805 บาท ต้นทุนรวม 884.4 ล้านบาท และโรคติดเชื้อที่ จมูก ปาก คอและกล่องเสียง DRG 03520 nose, mouth, throat and larynx diseases wo sig CCC 16,606 บาท ต้นทุนรวม 276.5 ล้านบาท ผู้ป่วยสูงอายุ (อายุมากกว่า 60 ปี) การผ่าตัดเลนส์ตา DRG 02060 Major lens procedures wo sig CCC มีต้นทุนเฉลี่ยต่อราย 16,203 บาท ต้นทุนรวม 597.2 ล้านบาท รองลงมาเป็นโรคหลอดเลือดสมองผิดปกติ DRG 01550 Specific cerebrovascular disorders exc TIA wo sig CCC 12,023 บาท ต้นทุนรวม 300.8 ล้านบาท และ โรคมะเร็งระบบย่อยอาหาร DRG 06691 digest malignancy w chemotherapy w min CCC 13,134 บาท ต้นทุนรวม 180.8 ล้านบาท
         สรุป การศึกษานี้ได้ทำการศึกษาต้นทุนการให้บริการรายกลุ่มวินิจฉัยโรคร่วม โรงพยาบาลที่รักษา และอายุของผู้ป่วย ต้นทุนที่เกิดขึ้นจริงในการดูแลรักษาผู้ป่วยกลุ่มวินิจฉัยโรคร่วมที่พบบ่อยในกลุ่มอายุเดียวกัน ต้นทุนจะไม่แตกต่างกันนักระหว่างโรงพยาบาล อันจะเป็นข้อมูลสำคัญสำหรับนักวิชาการ, ผู้ให้บริการ, นักวิจัย และผู้กำหนดนโยบาย เห็นความสำคัญและนำไปใช้ประโยชน์ในการคำนวณงบประมาณ รวมถึงการจัดสรรทรัพยากรให้หน่วยบริการอย่างเหมาะสมต่อไป

Abstract

          This study aimed to present the cost of inpatient services per diagnosis related group based on individual inpatient data. The data were obtained from 40 hospitals that voluntarily participated in the First Phase Cost per Disease Project over a four-year fiscal period (2018-2021), totaling 2,263,459 cases. The costs were from the provider's perspective and calculated using standard top-down and bottom-up methods. The analysis included the average cost per DRG and the total cost (average cost per case multiplied by the number of cases). Prior to the analysis, abnormal cost data were excluded, resulting in 2,191,117 cases (96.8%) for analysis. After further exclusion of the COVID-19 pandemic admissions (DRG 23540, 04520, 04521), the top three most frequent DRGs were presented. 1) DRG 15540 Newborn admission weight >2,499 gm without procedure without significant cost and clinical complexity had average cost of 5,192 baht per admission and a total cost of 599.2 million baht. 2) DRG 14010 Caesarean delivery wo sig CCC with an average cost of 18,954 baht and a total cost of 1,248.1 million baht. 3) DRG 14500 Vaginal delivery wo sig CCC with an average 15,810 baht and a total cost 889.1 million baht. For large hospitals (300 beds or more), the average cost per admission for DRG 15540 was 5,187 baht, for DRG 14010 18,704 baht, and for DRG 14500 15,465 baht. For small hospitals (less than 300 beds), the average cost for DRG 15540 was 5,216 baht, for DRG 14500 17,556 baht, and for DRG 06570 Infectious gastroenteritis age > 9 wo sig CCC was 5,654 baht. For pediatric patients (from newborns up to 15 years old), the costs for three most common DRGs were 5,192 baht per admission and a total cost of 599.2 million baht for DRG 15540, 5,360 baht per admission and a total cost of 114.4 million baht for DRG 06570, and 8,059 baht per admission and a total cost of 121.4 million baht for DRG 03520 nose, mouth, throat and larynx diseases wo sig CCC. For working-age patients (aged 15 to 60 years), the average cost per admission for DRG 14010 was 18,954 baht and a total cost of 1,246.1 million baht, with DRG 14500 15,805 baht per admission and a total cost of 884.4 million baht, and DRG 03520 16,606 baht per admission and a total cost of 276.5 million baht. For elderly patients (above 60 years old), the average cost per admission for DRG 02060 Major lens procedures wo sig CCC was 16,203 baht and a total cost of 597.2 million baht, DRG 01550 Specific cerebrovascular disorders exc TIA wo sig CCC average cost 12,023 baht per admission and a total cost of 300.8 million baht, and DRG 06691 digest malignancy w chemotherapy w min CCC with an average cost per admission of 13,134 baht and a total cost of 180.8 million baht. In summary, the present study examined the cost of inpatient services with the focus on DRG, hospital groups, and patients’ age. The actual costs incurred in caring for most common DRGs for narrow age range varied insignificantly by hospital groups. This information is valuable for academics, healthcare providers, researchers, and policymakers. It underscores the importance of using this data for budgeting and resource allocation to ensure optimal healthcare service provision in the future.
